If i can't see myself i can't point you in the right direction. If you don't have any error, something else is the issue. Try to deactivate all plugins, and see if this will work. Leve only this plugin.
This is strange because this plugin has some minimal code in that file. If is an error in the file, then some code that is above of that error should be displayed. Check the file permissions, but I doubt that is the issue.
Other page settings from other plugins are loading?

From the first image that you posted I see that the style is not loaded so somewhere is an issue.

i have just found problem, and just fixed. I have one more question for how i can use it for registration without email and password? only name and number?

First, check to see if you have some js errors.
In Firefox Ctrl+Shift+K
You can track in the console if any error is triggered when you click the close button or if you already have a js error on the site.

The ajax request is done correctly? The pop-up is closed only after the ajax calls is made with succes.

Hi, Did u try to login from mobile devices?
Because from my mobile i couldnt login, opened account kit in new tab and, on web it opens popup, in settings fb developer i have write redirect domain , but didnt redirect for back, and so didnt login to site
